Intramural Basketball Interest Survey: Grades 4–6
The Utica City School District is exploring a potential after-school Intramural Basketball Program for 4th, 5th and 6th-grade students at our elementary schools.
Families are invited to complete a short interest survey to help us assess participation and plan program details. Completing the survey does not commit your child to participate.
Please submit the survey by Friday, February 13, 2026. Families may complete either the paper survey sent home or the electronic survey link, not both.
